How to get "The Insane" title.

1. “Bloodsail Buccaneers” reputation - honored requied
Set BootyBay faction “At war” in your reputation list then go to Bootybay and kill guards around. This will increase reputation with Bloodsail Buccaneers, do this until honored then go to cords 30:90 and complete the quests from npc Fleet Master Firallon.

2. “Steamwheedle Cartel” - exalted required
Go to Dire Maul located in Feralas.
If you don’t have a rogue or any other devices to open doors you need Crescent Key - the one key that opens all Dire Maul’s doors. For that go to the entrance located on East on your map (the only one you can get in without a key).
On your left as you entered there’s an imp called Pusillin, talk to him and follow him until you can kill him and loot the key (before talking to him mark him because he runs faster than you and gets off your line of sigh).
Now you can start increasing rep with Cartel. Get inside Dire Maul North and find a goblin named Knot Thimblejack and repeat the quest “Free Knot” until exalted (1750 / 1925 rep - humans). To complete this quest you need to kill ogres inside Dire Maul and loot Gordok Shackle Key. You can only carry 1 so try to agro as many ogres you can carry and kill them near Knot, loot one key/complete quest and so on. If any Pristine Black Diamond drops make sure to loot all since we need them for the next faction.

3. “Shen'dralar” - exalted required
In Dire Maul West there’s a npc called Lorekeeper Lydros which gives 3 repetable quests but only 2 of them are working:
a) Libram of Focus - Requirements:
- Libram of Focus x 1
- Pristine Black Diamond x 1
- Large Brilliant Shard  x 4
- Skin of Shadow  x 2 - drops from Risen Construct’s in Scholomance (Western Plaguelands)
b) Libram of Protection - Requirements:
- Libram of Protection x 1
- Pristine Black Diamond x 1
- Large Brilliant Shard  x 2
- Frayed Abomination Stitching - drops from abominations found near Lord Rivendare in Stratholme (Eastern Plaguelands).
Both quests give 2500 / 2750 rep - humans, you need a total of 42.000 reputation.
Pristine Black Diamonds drop from every instance in range of 55-60 (BRD, UBRS, LBRS, Strat, Scholo, DM and ZG, MC Trash).
Skin of Shadow  and Frayed Abomination Stitching drops 100% so get at last 1 stack of each to be sure. While farming Skin of Shadow  and Frayed Abomination Stitching in Scholo/Strat ask an Enchanter to party with you, blue items drop a lot so you’ll DE em and get your Large Brilliant Shard  required.
Librams (of Focus, of Protection) can be found in Dire Mual, you can only carry 1 of each so try again to agro elites closer to the npc that gives quests (be sure you don’t have any of this two librams in your bank when you go in Dire Maul).

4. “Ravenholdt” - exalted required
Kill mobs in Durnhold Keep (Hillsbrad Foothills) until revered. Once revered you won’t get any rep from killing mobs. Go now to 73:23 coords in Hillsbrad Foothills, follow the hidden path up in the mountain.  Find npc Fahrad, he gives the repetable quest “Junkboxes Needed” (375  / 410 rep - humans): requires 5 x Heavy Junkbox.
Easiest way to get these is by pickpocket lvl 55+ mobs Scarlets, Skeletals, Zombies  in WPL (you need 280 / 260 boxes - humans). Now get a rogue and start farming, once you got all boxes mail them to your char. Ask some engineer to sum his MOLL-E near the npc that gives the quest and hurry (you only have 10 minutes until MOLL-E disappears).  
To gather your boxes faster I advise to stick at 2, 3 areas/groups of mobs, pickpocket 1 area and kill them all, go to second area and do the same, until you finish the second area mobs in first area will resp. It takes like 5 minutes for a mob to respown and probably double the time for a junkbox to be available to pickpocket it again on the same mob that you didn't killed.

5. “Darkmoon Fire” - exalted required
Now you need herbalist + inscriptioner. Clearing Burning Steppes takes around 30 minutes and you get like 2 full bags of herbs. Make as much Darkmoon Cards you can and transform em in Decks which you turn on to Darkmoon Faire (you get 1750 / 1925 rep for each deck).
