2) Ur blood tank spec is completely useless to be honest, in 2 min of googling i found a better one

3)No macros

4) Completely useless add ons espicially for pve.

5)No one can vouch for you

7) Its your 2nd application and you really havent even tried.

No alts, so lacks experience in raiding, since pugs on this server only do 1st 3 bosses

There's nothing left to say.

So I will try to point out mistakes that You made with your class:
Your blood tanking spec is not so good (as Dothem said), major mistake is taking frost talent that increases off hand damage, since You want to tank with 2h weapon.

Mark of Blood is just a talent waste. Every boss will damage You for more than 4%.

Spell Deflect talent is a situational one. It deflects only spells that are casted directly to You, so this wont help You against AoE spells.

Scent of Blood 1/3 will give You so much RP, so I would take that.
Epidemic 2/2 helps You handle Your rotation easier.

Every DK needs some add-on for Rune tracking(built in one is just small and useless). I would recommend Archerus Runes.

Your pvp spec is almost perfect, I would just drop 4/5 Toughness and take 2/2 Runic Power Mastery and 2/2 Endless Winter.
