I wouldn't say you've put enough effort in application form, just short answers with nothing to read actually.
Your UI is bad, as well.

Though, appareantly you're an old member of this guild, and I should be negative about it, since I'm yet a new one. Neither I know the reason why have you left this guild / or got kicked.
